South Central LTC RISE
Long-Term Care (LTC) Facilities have experienced severe devastation from the COVID-19 pandemic. The challenges of caring for out most vulnerable populations in this setting, while managing the threat of COVID-19, creates extremely challenging and stressful circumstances.
It is crucial that LRC facilities receive continued support as they both battle the COVID-19 pandemic and work to recover and rebuild.
The Southcentral LTC-RISE team at Penn State Health is responsible for coverage of 13 PA counties in this region including: Adams, Bedford, Blair, Cumberland, Dauphin, Franklin, Fulton, Huntingdon, Juniata, Lebanon, Mifflin, Perry, and York.

Long-Term Care Resiliency, Infrastructure Supports and Empowerment
The LTC Resiliency, Infrastructure Supports and Empowerment (RISE) program is a partnership between the Pennsylvania Department of Health, Pennsylvania Department of Human Services, Pennsylvania Emergency Management Agency and four healthcare organizations. The healthcare organizations have established partnerships covering six regions across Pennsylvania.
